We provide basic care coordination for all of our clients at no cost, along with enhanced levels of care coordination that can be tailored to need and selected when, or if, more is needed. Care Coordination Options
Care Coordination levels are fee-based
Support Provided to All Clients:
- Admission Visit with creation of Service Plan
- 15 Day Follow Up Virtual Call
- 30 Day Follow Up Virtual Call
- Quarterly Check In Monitor
- Annual Service Plan Visit and Reassessment
Level I Support:
This service option allows a client to request any of the care coordination services listed below discreetly on an as needed basis. These service options are stand alone and can be requested as needs arise. These services can also be used in conjunction with our Level II and Level III ongoing care management monthly services.
- Per Hour Coordination Services – used as needed
- In Person Hospital/Nursing Facility Check in Visits
- Medication Management Visits
- Accompaniment to Medical Appointments
- Hospital/Rehab to Home Transitions
Level II Support:
This level of support is appropriate for clients who are in general good health, are able to understand and direct most aspects of health and life management but want general support overall or support in specific areas to assure continued independence.
- One in-person visit each month
- Two hours of dedicated care coordination based on established care support plan
- 24/7 On Call Availability – for any crisis or emergent issue that arises
Level III Support:
This level of support is appropriate for clients who are declining in health or who have suffered a medical event, clients who are overwhelmed by managing their health or life management responsibilities, clients with complex needs impacting their ability to continue living independently.
- Two in-person visits each month (one can be accompaniment to medical appointment)
- Four hours of dedicated care coordination based on established care support plan
- 24/7 On Call Availability – for any crisis or emergent issue that arises

About Superior Home Care
As a licensed Home Care provider by the Pennsylvania Department of Health, Superior Home Care can provide services under the following programs:
- Pennsylvania Department of Human Services for both the ODP Waiver and the Act 150/Attendant Care Waiver services
- VA to provide Aide and Attendant services
- PA Office of Long Term Living’s Community Health Choices Program (AmeriHealth Caritas, PA Health & Wellness, UPMC)
- SW-PA Area Agency on Aging to provide Options In-Home services
- Highmark Health Plan – for qualifying lines of coverage that provide hospital to home transitional care
- Other insurances (such as long-term care insurance) can be considered based on insurance policies
Services outside of the above programs can be provided on a self-pay basis.
