Personal Care & Assistance covers a broad range of support services and tasks that assist with essential daily activities and are customized to meet a person’s specific and changing needs. Tasks can cover a spectrum of support from companionship, socialization and accompaniment into the community to homemaking, shopping and meal preparation thru all aspects of personal hygiene and grooming. Service plans can include a combination of these different types of tasks or may be designed for only one specific task, such as assistance with medical appointments.

Understanding the Personalized Support Plan Process

Each client we serve receives an initial in-person comprehensive assessment. This assessment helps us understand clearly where support is needed and wanted and what a person’s goal for care is, to ensure our care is designed to deliver the optimal care experience. We create the Support Plan together, in collaboration. All tasks to be completed at each visit are defined in the Support Plan. Any special skills needed, or caregiver preferences, are included in the planning process, so the right caregiver is matched to provide care ongoing. The Support Plan can, and most likely will, change and adjust over time as a person’s needs change. Each caregiver working with a client is required to follow the Support Plan ongoing.

As a licensed Home Care provider by the Pennsylvania Department of Health, Superior Home Care can provide services under the following programs:

  • Pennsylvania Department of Human Services for both ODP Waiver and Act 150/Attendant Care Waiver services

  • PA Office of Long Term Living’s Community Health Choices Program (AmeriHealth Caritas, PA Health & Wellness, UPMC)
